A Complete Clinical Guide for Sandhivata, Aamavata & Vatarakta

Arthritis, from an Ayurvedic perspective, is not merely a joint disease. It is primarily a disorder of Vata dosha, often complicated by Ama (metabolic toxins) or Rakta dushti (blood vitiation).

Painkillers suppress symptoms. Ayurveda corrects the root pathology — Agni, Ama, Vata, and Dhatu depletion.

Arthritis manifests mainly in three forms:

  • Sandhivata → Degenerative (Vata dominant)

  • Aamavata → Autoimmune inflammatory (Ama + Vata)

  • Vatarakta → Gout type (Vata + Rakta)

Management depends entirely on correct identification.


1️⃣ Sandhivata (Osteoarthritis Type)

Pathogenesis

Aggravated Vata enters the joints (Sandhi), causing:

  • Dryness

  • Crepitus

  • Pain worse in cold

  • Degeneration of cartilage (Asthi dhatu kshaya)

Inflammation is mild or absent. The core issue is dryness + degeneration.


Step 1: Vata Pacification

Daily routine:

  • Warm, freshly cooked food

  • 1–2 tsp cow ghee daily (if digestion permits)

  • Avoid fasting

  • Avoid dry snacks

  • Early sleep before 10:30 pm


Step 2: Internal Medicines

Maharasnadi Kwath
15–20 ml + equal warm water
Twice daily after meals
Duration: 8–12 weeks

Yograj Guggulu
1–2 tablets (250–500 mg each)
Twice daily after meals

OR (chronic severe stiffness)

Mahayograj Guggulu
1 tablet twice daily

If associated back pain:

Dashmool Kwath
15 ml twice daily after meals

If constipation present:

Triphala powder
3–5 grams at bedtime with warm water


Step 3: External Therapy

Daily:

  1. Warm sesame oil massage (10–15 minutes)

  2. Mild fomentation (5–7 minutes)

After skin dries:

Magnesium Oil Application

  • 6–10 sprays over affected joint

  • Light massage

  • Once daily (evening preferred)

Magnesium helps:

  • Relax periarticular muscles

  • Reduce neuromuscular irritability

  • Improve circulation

  • Improve sleep quality

Duration: minimum 6–8 weeks.


Step 4: Asthi Dhatu Nourishment

  • Sesame seeds

  • Warm milk (if digestible)

  • Sun exposure (Vitamin D)

  • Gentle strengthening exercises


2️⃣ Aamavata (Rheumatoid / Autoimmune Type)

Pathogenesis

Weak digestion → Ama formation
Ama combines with Vata → lodges in joints
Causes swelling, warmth, heaviness, stiffness.

This is a metabolic-inflammatory condition.


Phase 1: Ama Pachana (7–21 Days)

Diet:

  • Moong soup

  • Thin khichdi

  • Warm water sipping throughout the day

Avoid strictly:

  • Curd

  • Cheese

  • Bakery

  • Fried food

  • Cold drinks

  • Day sleep


Medicines for Ama Stage

Shunthi + Jeera Decoction
½ tsp dry ginger + ½ tsp cumin
Boil in 200 ml water → reduce to 100 ml
Twice daily before meals

Sanjeevani Vati
1 tablet (125–250 mg) twice daily before meals

OR

Aamvatari Rasa
1–2 tablets twice daily after meals


Phase 2: Anti-Aamavata Medicines

Simhanad Guggulu
1–2 tablets twice daily after meals

Rasnadi Kwath
15–20 ml + equal warm water
Twice daily after meals

If systemic inflammation high:

Dashmool Kwath
15 ml twice daily

Duration: 8–16 weeks


External Therapy

During acute swelling:

  • Avoid heavy oil massage

  • No heat therapy

Magnesium oil:

  • 4–6 sprays

  • No strong rubbing

  • Once daily

After swelling reduces:
Abhyanga → mild swedana → magnesium oil


Basti Therapy (Important)

After Ama reduces, Vata must be stabilized with:

  • Vata-Aamavata Basti protocol

Basti prevents long-term joint deformity.


3️⃣ Vatarakta (Gout Type)

Pathogenesis

Vata + Rakta dushti
Often triggered by alcohol, meat, metabolic disturbance.

Symptoms:

  • Sudden severe pain

  • Redness

  • Burning

  • Big toe involvement


Step 1: Remove Triggers Immediately

Avoid:

  • Alcohol

  • Red meat

  • Fermented food

  • Very spicy food

  • Excess sour

Hydration:

  • Warm-normal water

  • No ice cold drinks


Step 2: Internal Medicines

Guduchi Kwath
15–20 ml + equal warm water
Twice daily after meals

OR

Guduchi Ghana Vati
1–2 tablets twice daily

Gokshura powder
3–5 grams twice daily

OR

Gokshuradi Guggulu
1–2 tablets twice daily

Triphala
3–5 grams at bedtime


External Therapy

During acute attack:

  • No heat therapy

  • No strong massage

Magnesium oil:

  • 4–6 sprays

  • No rubbing

  • Once daily

After acute stage:
Mild oil massage + magnesium can resume


Universal Principles for All Arthritis

✔ Maintain bowel regularity
✔ Maintain ideal weight
✔ Fixed routine
✔ Early sleep
✔ Daily gentle movement
✔ Long exhalation breathing (10–15 minutes daily)


Where Magnesium Oil Fits Clinically

Magnesium acts as:

  • Neuromuscular relaxant

  • Vata pacifier

  • Pain-modulating mineral

  • Sleep-supportive agent

Best time:
Evening, after bath or fomentation.

Avoid:

  • Broken skin

  • Severe renal impairment (caution)


Core Ayurvedic Insight

Arthritis is not a joint disease.

It is:

  • A Vata disorder

  • Often aggravated by Ama

  • Sometimes complicated by Rakta dushti

  • Rooted in weak Agni

I have swelling in my right hand thumb stiffness and and sharp pain from last 1 and half month
I m taking ayurveda tablets r
Rasnerandadi tablets
Balya churna ( 200 ml water boil 1/2 tsp till 100 ml)
Triphala tablets.
But till now I didn’t get any relief from pain
